Social Media Use Agreement (Ohio)

Social Media Use Agreement Instructions:

In the opening paragraph enter the date as day, month, then year. Next enter the parent’s name and then the child’s name.

A note to remember, if there are any sections provided in the Agreement that you do not want to apply or wish to use at this time enter not appliable and move on to the next section with your child.

Section I– Social Media Usage:   check off any rules that you want to apply, if there is a rule that you do not see in the agreement, we have provided you with empty space to fill in the rules you want.

Section II – Social Media Conduct: check off any rules or codes of conduct and sharing of content on any social media platform/network that you wish to apply.

Section III- Consequences: Here you can state any consequences that will be applicable if any of the rules or guidelines regarding social media use and/or conduct are not followed.

Section VI -Additional Terms: Enter any additional terms that you deem important or did not see included here.

Section VII: This is the signature section; each party will sign the agreement and print their name to make the agreement binding.
