Top of the Letter enter the Landlord’s name and Address
Enter the date of this letter.
Next enter the applicant’s name and address.
Enter the name of the person the letter is being address to on the Dear line.
Enter the address of the premises that applicant put in an application for.
Check off which one of the credit reporting agencies you used. You are required to do so under the FCRA of 1997.
Sign the letter.
